We will size and/or rotate the text to fit the tag, but please be brief. We recommend no more than 5 lines per tag, with 15 to 18 characters/spaces per line. If you need more room, please consider purchasing additional tags.
Make your selection from the drop-down box for "Chain Size/Key Ring".
To order multiple tags with different information: return to this web page and repeat steps 1 and 2.
Click 'CHECK OUT' to view the shopping cart and complete your purchase.
You may request a digital proof of your medical tag be e-mailed to you
before your order is shipped. During check-out, use the "Notes/Instructions:" box in the "Shipping
Address" section of the shopping cart to request a digital proof; be sure to include
your e-mail address.
For example: "Please send a digital proof for this order to myname@email.com"
